SGMC Primary Care – Connell is the continuity clinic for SGMC Health’s Family Medicine Residency Program, where patients receive comprehensive primary care from resident physicians who serve as their primary provider throughout training. Patients of all ages are welcome—from newborns and children to adults and seniors—and can establish long-term relationships with their care team while benefiting from the oversight of experienced attending physicians. The clinic offers preventive care, sick visits, chronic disease management, pediatric care, and family appointments, allowing multiple family members to be seen during the same visit for added convenience. This collaborative model provides high-quality, patient-centered care while helping train the next generation of family medicine physicians.
