IMPORTANT:
When you are filling out the form above, make sure to include the patient's first and last name so our volunteers can make sure your Greeting Card is delivered to the right individual. Cards will be delivered Monday-Friday from 8 a.m. to 12 p.m.

Those cards that are completed after 12 p.m. will be delivered the next day and those sent on the weekend will be delivered the following Monday.

Cards will not be delivered on Thanksgiving Day, the Friday after Thanksgiving, Christmas Day and New Year’s Day, as our office will be closed. Cards completed during this time will be delivered the following Monday.

Please note: The content of your Greeting Card message is not confidential and will be seen by members of our Auxiliary.
